• UChicago Medicine uses ASHP's Pharmacy Online Residency Centralized Application Service (PhORCAS) across all pharmacy residency training programs.

  • More information about PhORCAS can be found by visiting www.ashp.org/phorcas.

• Each application requires three letters of recommendation, a letter of intent, academic transcripts, and curriculum vitae.

• PGY-1 Community-Based Supplemental Application:

  • Please complete the following supplemental application and upload this document to your PhORCAS application.​

  • This supplication application is required ONLY for the PGY-1 Community-Based Pharmacy Residency Program

  • The supplemental application must be received by the program's deadlines as stated in the program brochure

  • Supplemental Application Form (updated 11/2023)

Work Authorization:

Applicants must be eligible to work in the U.S. without company sponsorship, now and in the future, for employment-based work authorization.  No sponsorship will be available upon completing Residency Program.
